For any believed infringement of a copyrighted work via The TRIMEDX Foundation’s Website, please submit the following information as set forth below:

  1. Identification of the copyrighted work or works that are being infringed;
  2. Location where the infringing material can be found;
  3. The name, telephone number, and e-mail address at which the copyright holder may be contacted;
